The form takes just a few minutes to complete and ensures we receive all the information needed to register you safely and efficiently.
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
Summary Care Record (SCR)
The NHS Summary Care Record (SCR) is an electronic record that contains important information about your health, including any medications you are taking, allergies you have, and any previous adverse reactions to medicines. This information can help healthcare professionals provide you with safe and effective care, particularly in urgent or emergency situations.
Your Summary Care Record can be accessed by authorised healthcare professionals involved in your care anywhere in England. Whenever possible, healthcare staff will ask for your permission before viewing your record.
Having a Summary Care Record means that, if you become unwell or require treatment away from your usual GP practice, clinicians can quickly access key information about your health to support your care.
Your Choice
Our practice supports the use of Summary Care Records; however, having an SCR is your choice.
- If you are happy to have a Summary Care Record, you do not need to take any action. A record will be created automatically.
- If you do not wish to have a Summary Care Record, please complete an SCR Opt-Out Form and return it to the practice along with your registration documents.
You can change your decision at any time by contacting the practice.
Children Under 16
A Summary Care Record will normally be created automatically for children under the age of 16 unless a parent or legal guardian chooses to opt them out.
If you are a parent or guardian and believe your child is mature enough to understand what a Summary Care Record is, we encourage you to discuss this information with them.
